In our practice, we follow a Direct Care model, which means we do not bill insurance for office visits. Instead, patients pay a monthly membership fee that covers the cost of the doctor’s time and ongoing access to care. By removing the constraints of insurance billing, we can focus entirely on you—your goals, your health history, and your path to wellness.

Terms & Conditions

At Awaken Health, we are committed to providing personalized, relationship-based care rooted in a holistic, functional medicine approach. Please read the following terms carefully, as they serve as both our patient agreement and practice policy.

Membership Selection

Membership tier is selected during your initial Meet & Greet or first consultation based on your current health needs and goals. Tiers determine your level of access, visit frequency, and communication options. After completing the initial three-month commitment, you may request a tier change. All tier changes must be reviewed and approved by your doctor to ensure continuity and alignment of care.

Membership Commitment

We require a 6-month minimum commitment because it often takes that long to complete diagnostics, review results, create a personalized plan, and begin evaluating your response. This ensures we deliver thoughtful, effective care—not quick fixes. After this period, you may cancel your membership at any time with no penalty. Tier changes may only be made after the initial three-month commitment and require physician approval. There is no long-term contract; we aim to offer flexibility while maintaining consistent, high-quality care.

Monthly Payment Structure

The monthly membership fee is designed to spread the cost of the doctor’s care over time, rather than charging a larger fee per visit. This model provides consistent income that helps keep our patient panel smaller and support our mission: delivering high-quality, accessible, and personalized care. Please note that paying monthly does not guarantee or require monthly visits—care is provided as needed, based on your individual health goals and concerns. Membership includes a wide range of perks such as extended office visits, no wait times, a whole-person approach to care, and a genuine therapeutic relationship with your doctor—plus unlimited urgent care visits, same/next-day availability by appointment, direct text access to your doctor, and exclusive discounts on supplements, IV therapy, medications, labs, imaging, and more.

Subscription Policy

Monthly payments are non-refundable and cannot be paused, held, or suspended. Canceling your membership ends access to all services at the conclusion of your current billing cycle. Once your membership is canceled, we can no longer serve as your primary care provider. This means we cannot manage your care, prescribe medications, or provide refills. If you choose to rejoin in the future, a $199 re-registration fee will apply.

Texting & Communication

You may contact your doctor via secure messaging for non-urgent medical concerns. We do our best to respond as quickly as possible, but standard response time is 1–2 business days. If a message is marked “URGENT,” we will prioritize it during business hours. Texting is not appropriate for emergencies or highly time-sensitive issues. We opened our clinic to gain the autonomy and flexibility to prioritize what matters most—family. While we’re committed to providing excellent care, we want to clarify that we are not a concierge practice. While we do our best to support our members, weekend and after-hours availability is not guaranteed. Thank you for understanding as we balance personalized care with the values that guide our practice.

Medication Refill Requests

Please request prescription refills at least 3 days in advance. This allows time to safely review your history and process the request. Requests made with less notice may result in delays. We cannot provide refills after membership has been canceled.

After-Hours Care

We aim to help you avoid unnecessary urgent care visits. While we strive to be available, after-hours responses are not guaranteed. For emergencies or immediate care needs, please call 911 or visit the nearest urgent care center or emergency room.

By enrolling in membership with Awaken Health, you acknowledge and agree to the terms outlined above. These policies are designed to support transparency, accessibility, and a collaborative approach to care.
